Wag kang iiyak kahit mahalata mong masaya siya sa piling ng iba. Ni ang malungkot, iwasan mo. Malay mo, hiniling din niyang maging happy ka kahit na alam niyang hindi na pwede kasi wala na siya. Matagal kong tinagong mahal kita. Ginawa ko ang lahat para sayo. Binigay ko na pati puso at kaluluwa ko pero hindi mo pa rin ako napansin. Alyssa Eve Carl What is ITP? ITP stands for “immune thrombocytopenic purpura.” This is usually a self limited blood disorder of childhood associated with low platelets. Platelets are also called “thrombocytes.” They are the sticky cells in the blood which are important in clotting and help to stop bleeding. Children with ITP develop easy bruising and bleeding. Bruises are from bleeding under the skin. These bruises are also called “purpura.” “Petechiae” refers to the red, freckle-like rash resulting from bleeding from tiny blood vessels in the skin or membranes in the mouth. The body’s blood producing organ is the bone marrow. This is the spongy, reddish material inside the bones. Platelets, in addition to both red and white blood cells are made there. White blood cells help us fight infection and provide us with immunity. Red blood cells carry oxygen throughout the body. In ITP, there is nothing wrong with the bone marrow’s ability to produce the blood cells, including platelets. ITP is NOT cancer. So What’s the Problem? This disorder results from decreased platelet survival in the body even though there is normal or increased production of platelets by the bone marrow. The “immune” part of the title comes from our knowledge that the immune system suddenly makes an “antibody” which attaches to the platelets and causes them to be destroyed by the spleen. The spleen is an organ in the left upper abdomen which acts as a filter for the body’s circulating blood. The spleen’s job is to detect, then filter out and destroy abnormal or old cells. The spleen is also an important part of a person’s immune and infection fighting system. The problem in ITP is that the platelets are covered with in “antibody” that the spleen detects as foreign, and therefore removes the coated platelets from the circulation. So, no matter how many great looking platelets the bone marrow can produce, if they are coated with the ITP antibody, the spleen will do it’s best to destroy as many of them as possible. How Serious is ITP? When your child’s platelet count is low, they will have an increased tendency to bleed. The lower the platelet count, the greater the risk for both internal and skin bleeding. In the majority of children, ITP goes away with or without treatment in less than six months. Normal platelet count: 150,000 to 450,000 (no restrictions for activity) OK platelet count: Above 80,000 (few restrictions for activity) Watch for bleeding and bruising when platelet count is below 50,000 (will require some activity restrictions for safety) What If It Doesn’t Go Away? There are two types of childhood ITP. The “acute” type usually resolves in six months or less. The majority of childhood ITP cases are acute. The “chronic” form of ITP can last years. A small percent of acute patients will have a recurrence of low platelet counts (thrombocytopenia) during a viral illness for up to a year after the original diagnosis. Chronic ITP is more likely to occur in children who are more than ten years old at the time of diagnosis. What Treatments are Available? The treatment plan for your child will be a very individualized decision made by you and your hematology doctor. Many factors such as your child’s physical condition, platelet count, activity level, and anxiety level will be considered. All of these treatments are aimed at controlling symptoms and bleeding. They do NOT cure the child of ITP. The goal of treating ITP with medications is only to increase the platelet count for a long enough period of time for the body to rid itself of the problem. Some of the options are outlined below: 1. Activity restrictions and frequent monitoring by medical staff. This involves frequent physical examination (possibly weekly) as well as a blood test to measure the platelet count. 2. Medication called Prednisone is taken orally at home for a short period (usually 10-14 days). This medication is a steroid which causes the spleen to remove less of the antibody coated platelets. Before your child is started on this drug, usually a bone marrow test is done to make sure that the “blood factory” is in proper working order. As with any drug, there is the possibility of allergy and sensitivity reactions. Prednisone can cause an increase in the appetite, stomach irritation, water retention, high blood pressure, high blood sugar, and irritability. Your doctor or nurse practitioner will discuss the bone marrow test and the steroid medication and possible side effects with you. 3. Medication called IVIG is given as an infusion through a vein over several hours for one or two doses. This is done in the hospital or the outpatient hematology clinic. The mechanism by which IVIG works is not completely known. Most likely, IVIG causes the spleen to remove less of the antibody coated platelets. It may also remove some of the abnormal antibodies made against the platelets. IVIG can cause allergic reactions, headaches, and discomfort from having to puncture a vein to infuse it. The effects of the IVIG are usually seen within 1-4 days after administration. The increased platelet count after ivig may last several weeks. The side effects of allergy and headache are usually only associated with the duration of the actual infusion. IVIG is a blood product. 4. Intravenous (IV) Anti-D is also a blood product given by vein to stop the destruction of platelets. It can be given quickly over a few minutes in the hospital or outpatient clinic. However, your child will need to be observed for up to 8 hours to monitor for acute anemia (hemolysis) and other complications. It is only helpful for patients who have an Rh positive blood type (example: B+, A+, AB+, O+) and for ITP patients who have not had their spleen removed. The elevation in platelet count may last several weeks. The side effects are similar to, but probably less common than those reported with IVIG. An additional side effect is anemia in some patients. This may require extra blood test monitoring. If your child has a low hemoglobin (anemia) or is actively bleeding, this medication may not be appropriate. 5. Splenectomy – this means surgical removal of the spleen. Splenectomy is usually reserved for chronic patients who are at greater risk for bleeding. Rarely it can be needed emergently for the treatment of serious bleeding, like brain hemorrhage. The spleen also acts as a filter for germs or bacteria. Therefore, children who have their spleen removed are at increased risk for some infections. To minimize this risk, the child must take an antibiotic by mouth twice a day, every day, for life. 6. Rituximab – this is also usually reserved for chronic patients and is sometimes used in acute ITP. This is an antibody that destroys the B lymphocytes in the blood, which are the main cells making the ITP antibody. It is given as an IV infusion. Side effects will be discussed by your child’s hematologist. What Should I Look For? 1. Check your child’s skin for bruises or petechiae during the bath. 2. Assist your child with toileting or stress to them the importance of reporting blod in the bowel movement or urine immediately. Blood in the urine may be red or brownish (tea or coke colored). The same is true for blood in the vomit. Blood in the bowel movement may look red or black like tar. 3. Watch for bleeding from the mouth or nose, especially during dry air season. Check the bed linen for signs of bleeding while sleeping. Encourage only gentle nose blowing, never closing off one side to blow the other. Use the softest toothbrush possible. Avoid use of dental floss until the platelet count is up. 4. Report any blows to the head, belly, or back to the medical staff promptly. What Else Should I Do? 1. Avoid giving your child any medications until discussed with your doctor or nurse practitioner. Many over-the-counter pain, cold, and cough medicines can affect your child’s platelet function. Aspirin or any medicine containing salicylates (like Pepto-Bismol and Alka Seltzer) should be avoided. Ibuprofen and strong antihistamines should also be avoided. Acetaminophen products (Tylenol, Tempra, etc) ARE SAFE to give your child for fever and pain whil the platelet count is low 2. Encourage plenty of fruits , fruit juices, water, and fiber foods to avoid constipation. Hard bowel movements, enemas, or suppositories can cause rectal bleeding. If needed, a stool softener can be recommended by your doctor or nurse practitioner. 3. Use plenty of toothpaste on a very soft bristled brush, and brush slowly and gently. You may need to assist or supervise your child. For infants, use a soft cloth or toothette swab without toothpaste. 4. Use lip balm to keep lips from drying and cracking. Use saline nose spray (Ocean spray, etc) if needed to moisten nasal passages during dry weather to help prevent nosebleeds. Room or house humidifiers can also be helpful. It can be helpful to run a soft wash cloth under water and stick it in a bag in the freezer so you will always have an “instant cold pack.” For nose bleeds, run the frozen cloth under cold water until soft and apply it with firm pressure to the nostrils. Use lotion on dry skin to avoid your child’s itching and scratching which could lead to bruises or petechiae. How Can I Protect My Busy, Active Child? Provide a safe environment for your child. Tell your child about these safety tops and why you must use them. Continuing some family structure and discipline is also important. Spanking or other physical discipline can cause bleeding. 1. No NOT allow your child to play roughly. This includes any contact sport such as wrestling, boxing, karate, soccer, football, and possibly basketball or baseball. A good rule of thumb when deciding if an activity is okay for your child is to allow anything within reason where the child’s feet never leave the ground. This usually excludes bikes, motorcycles, skates, skiing, diving, swings, money bars, tree climbing or gymnastics. 2. Avoid toys with sharp edges, drinking straws or popsicle sticks. 3. Pad the sides of cribs. For older children who may fall out of bed, put their mattress on the floor or put a side rail between the mattress and box spring. 4. For small children, pad the corners of furniture and counters that they might run into. Use a gate at the top or bottom of stairs. Clear obstacles such as tables or chairs that you frequently have to go around. DO NOT allow running through the house with obstacles or on the stairs. 5. Take up loose rugs. Us rubber no-slip mats in the shower or bath. 6. Older children should avoid shaving with razor blades. Electric shavers used gently are usually okay. 7. ALWAYS put a seat belt around yourself and your child. Us a car seat for smaller children. 8. Avoid travel in non-pressurized aircraft. What Should I Call the Doctor About? 1. If your child gets a cut or nosebleed, try a cold compress and firm pressure for 5-15 minutes. If you are unable to get the bleeding to stop after that, call the clinic or hematology doctor on call. To control a nosebleed, calm the child, sit them up, and apply firm pressure to the nostrils at the bridge of the nose with a cold compress continuously for five full minutes. 2. Call the clinic or hematology doctor on call AT ONCE for a blow to the head, a fall resulting in hitting of the head, or any event that causes your child to lose consciousness or awareness. Signs of Serious Injury or Bleeding in the Head: Headache which does not go away or worsens Vomiting Dizziness Confusion: child cannot say name or tell where they are Unusual sleepiness: difficult to awaken Change in or slurring of speech Eyes not moving together or looking the same way Weakness on one side of the body Loss of feeling, numbness or tingling in the hands or feet Stiff neck or back: arching of the back Seizures: uncontrolled body jerking or twitching Child reports inability to see or hear The good thing is that spontaneous serious bleeding in childhood ITP is rare. Also the risk of spontaneous bleeding is greater only when platelet count is less than 20,000 and usually when less than 10,000. Common sense precautions in avoiding hard contact/injury will go a long way in preventing bleeding. The most obvious bleeding seen in children with ITP is skin bruising and petechiae. These are not dangerous forms of bleeding by themselves. In 80% of children, ITP will go away by itself over 6 months.

Pauline James I read in my devotion this morning about a farmer who drove a boy around his large estate and bragged incessantly about his accomplishments. He told the boy of how he worked hard and bought that piece of land and still had so much money left over and how he worked hard to create the lush farm they were looking at from NOTHING! “Yes I did it all by myself from NOTHING”, he boasted. The boy looked at him with wide-eyed amazement, impressed beyond measure and said, “You did all this from NOTHING?” Yes, sonny, I did it all from Nothing!” he said with pride. The innocent boy, being marveled at all he saw said to him, ‘O, Wow…my daddy farms too but he needs seeds to grow his crops!” Somebody say “Ouch!” The next time you feel like bragging about your accomplishments remember that it was God who seeded you with gifts, talents, abilities and skills to do such a good job. You came into his world as a naked, hungry, open-mouth screaming baby but God has filled your life with so much seed so that you can be productive today. He sent you opportunities, ideas, innovation, good people to influence your life, prophetic words to stir you, bad experiences to provoke you, harsh critics to bring out the best in you, dreams, visions and an audience of friends and family to cheer you on and market your creativity. He anointed your life so that your cup “runneth over” and the anointing does “profit withal”. (1 Cor. 12 v 7) The devil is a liar; you could not do it without seeds. God seeded your nothingness and filled your life with abundance. I know you feel very special but the credit belongs to God. Everyone else knows it and God knows it too, so stop the bragging and give thanks! What if God should withdraw his seeds from the well-established acreage of your life? Can you imagine how everything would go into reverse and you would dry up on the vine? Somebody think about this and then start shouting! Every good gift and every perfect gift is from above, and cometh down from the Father of lights, with whom is no variableness, neither shadow of turning. (James 1 v 17)
